There are two options for entering data (see HEP Handbook for detailed instructions).

Preferred option: enter data using your mobile device.

Install the Survey123 mobile app. You can find it in Apple’s App Store, the Google Play Store, the Amazon app store, or the windows app store by searching for “Survey123.”
Once the app is installed, click here from your mobile device to download the 2023 data entry form. If prompted, choose “open in app” to launch the Survey 123 app and begin entering data.

Alternate option: enter data via your home computer. You can do so by entering data directly into the embedded form below.
